You do not have to use this form. Withdrawal is not bound to any particular form: an e-mail with an unambiguous statement is enough. The form exists because the FAGG requires us to make it available to you.

Deadline and consequences
The period is 14 days from conclusion of the contract. To meet the deadline it is enough that you send your statement before the period expires. We refund the amount paid without undue delay and at the latest within 14 days of receiving your statement, using the same means of payment.
If you expressly requested that the benefits apply before the period expired, and you used a benefit in that time, the value of the service already received must be reimbursed — this does not affect the withdrawal itself.
